We forget details, get distracted, make assumptions, and sometimes take shortcuts. These are normal human behaviors—not character flaws. Simply telling someone to “be more careful” is not enough. Human Performance focuses on identifying the conditions that increase the chance of error and putting controls in place before those errors lead to incidents.
